REPORTING FOR THE ADMISSION OF MD/MS COURSES 2021
As per the allotment done by Medical Council Committee, Directorate General of Health Services, Govt. of India, New Delhi, all the candidates who have been allotted MS/MD seats in the Mop-up Round of PG Counseling, in Institute of Medical Sciences & SUM Hospital are hereby directed to bring the following documents mentioned below:

Permanent / provisional Registration Certificate issued by MCI / State Medical Council.
College Leaving Certificate in Original
Conduct Certificate in Original
Migration certificate in original.
Aadhaar Card of the Candidate.
PAN Card of Candidate
10( Ten) No. of recent colour photographs.
02 sets of photocopies of all above documents.
Supportive documents for NRI Category (For NRI Seats Only)
a. Documents claiming that the sponsorer is an NRI (Passport, Visa of the sponsorer).
b. Relationship of NRI with the candidate as per the court orders of The Hon’ble supreme
Court of India in case W.P.(c) No. 689/2017- Consortium of Deemed 13 | P a g e Universities in Karnataka (CODEUNIK) & Ans. Vs Union of India & Ors. Dated 22-08- 2017.
c. Affidavit from the sponsorer that he/ she will sponsor the entire course fee of the candidate duly notarized
d. Embassy Certificate of the Sponsorer (Certificate from the Consulate).
